Demonstration Instrument Service Agreement Form CID Bio-Science, Inc. agrees to lend one factory-owned instrument to the person/company at the address listed below. Offer valid in North America only. Time and Condition Limitations: The loaner can be used for 10 business days, after which the instrument must be returned in like-new condition to CID Bio-Science, Inc. If and order is placed, the loaner may be kept until the new instrument arrives. The loaner must be returned within ten business days from the receipt of the new instrument. Location: The demonstration instrument must remain at the address provided below unless other arrangements have been made in writing. Sending the instrument elsewhere will result in charges being invoiced for the retail price of the instrument. Late Charges: After a 10 day grace period for return of the instrument, a $500 late fee will be assessed per month and invoiced monthly until the loaner is received at CID Bio-Science, Inc. Total invoiced amount shall not exceed the retail price of the instrument. Shipping Charges: CID Bio-Science, Inc. will provide the demonstration instrument at no charge and will pay for shipping to your location. Return shipping costs to CID Bio-Science, Inc. will be the responsibility, of the borrower and the shipment must be insured for the retail value of the instrument. Repairs: Any damages incurred to the instrument beyond normal wear and tear will be repaired and invoiced at standard CID Bio-Science service and repair rates.
